Case Management Services

  • Case Management Services: Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote community safety, quality of care and services provided, cost effective outcomes and essentially, a competitive integrated employment outcome.

  • Case Management Services includes :

      • Level One: Intake

      • Level Two: Needs Assessment

      • Level Three: Service Planning

      • Level Four: Monitoring and Evaluating

    • Case Management Services for Pre-employment transition coordination consists of:

        • All four levels list above and

        • Attending individualized education program meetings for students with disabilities

        • Working with the local workforce development boards, one-stop centers, and employers to develop work opportunities for students with disabilities, including internships, summer employment and other employment opportunities available throughout the school year, and apprenticeships

        • Working with schools, including those carrying out activities under section 614(d) of the IDEA, to coordinate and ensure the provision of pre-employment transition services

 

Our Case Management Services also includes attending court if a youth has to attend court hearings. We can also work with families if they are in need of transportation to and from a court hearing for a youth that we service. Case Management Services are included with all courses.

 

Case management serves as a means for achieving client wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation. The case manager helps identify appropriate providers and facilities throughout the continuum of services, while ensuring that available resources are being used in a timely and cost-effective manner in order to obtain optimum value for the client. Case management services are best offered in a climate that allows direct communication between the case manager, the client, and appropriate collaborative partners, in order to optimize the best outcome for all concerned.

 

Case notes will be created and posted in JTS so that the CCM/JPPS II will be able to review them with their clients. If we are not able to document in JTS, the case note will be created and emailed to the CCM/JPPS II to post in JTS. All collaborative efforts are to coordinate and be on one accord for the purposes of the youth.
