Case Management is a structured and collaborative process designed to assess, plan, facilitate, coordinate care, evaluate, and advocate for services to address the holistic needs of individuals and families. Pre-Employment Transition Coordination

Our Pre-Employment Transition Coordination services are designed to prepare students with disabilities, regular students, at-risk students, and young adults for meaningful employment opportunities. These services focus on equipping individuals with the skills, resources, and experiences needed to transition successfully into the workforce.

Court Support as Part of Case Management Services

Our Case Management Services extend to providing essential support for youth who are required to attend court hearings. This includes:

• Court Attendance: Accompanying the youth to court hearings to provide guidance, advocacy, and emotional support during legal proceedings.

• Family Assistance: Collaborating with families to arrange transportation to and from court hearings if needed, ensuring they can participate and support their child.

• Family Coordination: Our Family Coordination services ensure that youth and their families are fully prepared for court appearances, addressing critical areas to promote a respectful and professional presentation.

These services are integral to our comprehensive approach and are included with all of the courses we offer. By addressing the logistical and emotional needs associated with court appearances, we help youth and their families navigate the justice system with confidence and support.

Case management serves as a means for achieving client wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation. The case manager helps identify appropriate providers and facilities throughout the continuum of services, while ensuring that available resources are being used in a timely and cost-effective manner in order to obtain optimum value for the client. Case management services are best offered in a climate that allows direct communication between the case manager, the client, and appropriate collaborative partners, in order to optimize the best outcome for all concerned.

Case Notes Distribution to Collaborative Partners

Case notes will be promptly created and shared via email with collaborative partners who have an immediate vested interest in the youth's progress. This includes professionals such as probation officers, school officials, therapists, counselors, and clinicians. Sharing these updates ensures seamless communication, supports coordinated efforts, and fosters informed decision-making to meet the youth's needs effectively.

Hosting celebrations for youth provides a meaningful way to acknowledge their individuality, achievements, and cultural identities. These events create opportunities to honor significant milestones, such as birthdays, fostering a sense of belonging and self-worth. Celebrating cultural events allows youth to connect with their heritage, share traditions, and learn about the diverse backgrounds of their peers. The Future Forward Commencement Gala is a biannual graduation ceremony held in the spring and fall to honor the accomplishments of program participants. This prestigious event celebrates the journey of each graduate, recognizing their hard work, resilience, and readiness to embrace a bright future.

The gala is an elegant, after-five affair, requiring attendees to wear formal attire that reflects the significance of the occasion. Graduates, their families, mentors, and community partners gather in a sophisticated setting designed to inspire pride and joy. The evening features a gourmet dinner prepared by skilled chefs, offering a luxurious dining experience to complement the celebratory atmosphere.

The Future Forward theme symbolizes the graduates’ readiness to step boldly into the next chapter of their lives. It emphasizes progress, vision, and the pursuit of new opportunities.
